Can somone please tell me how can I see if a database is fragmented and how to avoid it ... I believe that to avoid one needs to set up the extents at the tablespace level, which means that if a table has more extents will be better ?

Database is composed of tablespaces and which one of them is hosting tables, indexes and other segments. Which one of those three you are intersted into? Not clear from your post.

Anyway, here are the scripts for monitoring the fragmentation:
